Talking Matters Bendigo: Engaging Parents Early to Prevent Long-Term Speech, Language and Communication Needs in Preschool-Aged Children

This article reports on qualitative research conducted to evaluate parents’ perspectives of their experiences of Talking Matters Bendigo (TMB), a screening programme initiated between health and educational professionals in regional Victoria to improve access to speech pathologists for parents of preschool-aged children with speech, language and communication concerns. Drop-in clinics are conducted in three Bendigo schools monthly. The programme is a collaborative partnership between the Victorian Department of Education and Training, Maternal and Child Health and ‘Off to an Early Start’ (City of Greater Bendigo), Bendigo Health and the disciplines of Speech Pathology and Education at La Trobe University, Bendigo. La Trobe Education (Honours) student researchers interviewed a group of 10 parents attending TMB using face to face interviews and collected data using an online survey after parents attended a session. Thematic analysis of the data was completed and inter-reliability checks were completed by two external La Trobe PhD students to increase reliability and validity. Results indicated parents were satisfied with the information provided by the speech pathologists and they reported that they intended to utilise this new knowledge at home with their children. This study provides preliminary evidence that novel service delivery options such as TMB can be successful in engaging parents early in health literacy so that speech, language and communication problems in preschool-aged children can be identified, managed and even prevented.

Aims and Method
To survey the attitudes of in-patients on an all-female ward to staff gender. All patients were invited to complete an anonymous questionnaire which was then repeated on a four-weekly basis for 4 months. Staff members were surveyed once within this period.
ResultsOnly 15% of patients (n = 52) wanted all staff members to be female, whereas 87% reported feeling comfortable with male staff; 51% would prefer a predominantly female, mixed-gender staff. All staff (n = 11) were in favour of mixed-gender staffing.
Clinical ImplicationsCurrent best practice for staffing single-gender acute in-patient units is unclear. This survey is in line with findings from other studies in that a large majority of female in-patients prefer to be in a unit with mixed staff.

Background
Expert clinical judgement combines technical proficiency with humanistic qualities.
AimsTo test the psychometric properties of questionnaires to assess the humanistic qualities of working with colleagues and relating to patients using multisource feedback.
MethodAnalysis of self-ratings by 347 consultant psychiatrists and ratings by 4422 colleagues and 6657 patients.
ResultsMean effectiveness as rated by self, colleagues and patients, was 4.6, 5.0 and 5.2 respectively (where 1=very low and 6=excellent). The instruments are internally consistent (Cronbach's alpha > 0.95). Principal components analysis of the colleague questionnaire yielded seven factors that explain 70.2% of the variance and accord with the domain structure. Colleague and patient ratings correlate with one another (r=0.39, P<0.001) but not with the self-rating. Ratings from 13 colleagues and 25 patients are required to achieve a generalisability coefficient (Eρ2) of 0.75.
ConclusionsReliable 360-degree assessment of humane judgement is feasible for psychiatrists who work in large multiprofessional teams and who have large case-loads.

You will note from Part I that obsessive-compulsive disorder, formerly called obsessional neurosis before the word neurosis was eliminated from usage, has the highest clinical utility score of the disorders within the neurotic spectrum. It is therefore not surprising that we have much clearer guidelines and evidence for treatment than for others within the spectrum. The arguments in favour of both drug and psychological treatments are strong and are often complementary across the range of clinical indications in OCD. There are some disparities between the UK and USA but most are matters of emphasis rather than fundamental disagreements. We need more evidence from studies on combinations of drug and psychological interventions as this is a very common position in clinical practice.
Introduction
Obsessive-compulsive disorder (OCD) is a relatively common, usually chronic and sometimes very disabling condition, which by convention excludes clinically similar syndromes caused by psychoactive drugs or by a general medical condition. It is characterized by obsessions, compulsions or a combination of both. Obsessions are recurrent and persistent ideas, images or impulses that cause pronounced anxiety and which are usually recognized by the person affected as being self-produced, and yet irrational. Compulsions are intentional repetitive behaviours or mental acts (rituals) performed in response to obsessions or in response to self-imposed rules which are aimed at reducing distress or at preventing unacceptable or anxiety-provoking outcomes. Compulsive rituals are usually recognized as unreasonable, pointless or time-wasting and are also usually resisted by the individual affected.

Biogeochemical signatures were compared in ‘living’ and ‘dead’ Parmelia sulcata samples with their oak bark substratum. Eighteen elements reached maximum concentrations in ‘dead’ lichens, at lower concentrations than reported from industrial regions. High N concentrations in ‘dead’ lichens confirm exceedances of critical levels established for deciduous woodlands, supported by alien algae and ‘nitrophytic’ lichen colonization. Negative δ15N values recorded in lichen samples indicate N originated mainly from ammonia. Less negative δ15N values in healthy samples near busy roads suggest local NOx accumulation by Parmelia. Higher δ15N values in bark may result from different processes. Twenty-eight elements reach higher concentrations in healthy lichens near roads carrying higher traffic volumes. Thirteen elements correlate positively with lichen δ15N, suggesting that δ15N is a powerful indicator of the balance between agricultural and vehicular N influx. Maximum Ca and Sr concentrations recorded in bark and their spatial distribution suggest a local geological origin. High concentrations of Ga, Ba, Pb and Ni bark contents testify to a previous pollution legacy, including that from petrol which carried higher lead concentrations than today. Mn concentrations are higher than reported from other studies and show no clear relationship with local roads. Mn is known to limit lichen diversity and health in coniferous forests in US and Germany, but not yet in deciduous woodlands. Current atmospheric conditions and the former pollution legacy must be understood to conserve epiphytes and for biomonitoring.

Conventional medical treatment is often ineffective in the management of Irritable Bowel Syndrome (IBS). Although controlled trials of psychotherapy and hypnotherapy have been shown to be effective, psychological interventions are not widely available. The aim of this study was to assess the feasibility of the clinical provision of combined group cognitive-behaviour therapy (CBT) and hypnotherapy delivered by 13 therapists using a standardized protocol. Attempts were also made to disseminate the programme by patient graduates. A session-by-session instruction manual was compiled and the protocol was tested in a pilot group. Thereafter a total of 158 patients in 23 different groups took part in the 16-session intervention. Attrition rate was very low. Significant improvements in both gastrointestinal and psychosocial symptoms were observed following the intervention. The results suggest that professionally led group psychotherapy and hypnotherapy is a feasible option for the treatment of sufferers of IBS and increased efforts should be made in the training and supervision of user facilitators.
